Kindergarten Registration

Students should accompany parents this year for screening & registration.  The teachers will gather information about student abilities during registration time.  If you have significant or developmental concerns regarding your child, please feel free to contact Mr. Cali ([email protected]) or Mr. Hooks ([email protected]) to discuss.

 

The dates for the screening/registration will be February 27, 28 & March 1.  Please contact the office at 724-763-5299 to schedule your time.  If you missed the screening dates, please stop by the office to pick up a packet or use the electronic forms below.  Screenings will be done during the 1st week of school.

 

Forms (found below) can be electronically completed, saved with your child's first & last name and sent to [email protected] or [email protected]. with the subject line of Kindergarten Registration in advance or you may complete, print, and bring with you to screening. If you prefer a paper copy you can print and complete it or stop in the school office to pick one up in advance.  Please bring the additional documentation needed (birth certificate, immunizations, proof of residency, and most recent dental and physical exam if available) with you to registration/screening and pick up a Kindergarten Readiness Bag for your child.

 

Electronically completed forms must be submitted to the office by the end of the day Friday,  February 24th.
